Cancellation Policy
All cosmetic patients must cancel 3 business days prior to their appointment or there will be a $95 fee. All dermatology patients must cancel 1 business day prior to their appointment or there will be a $65 fee.
Patient Forms
All new patients must have their paperwork filled out PRIOR to their appointment time and should still arrive 10 minutes early in order to be seen as scheduled. All needed paperwork is located below. Should your paperwork not be filled out, you will be asked to reschedule.
