## How virtual check-ins work

Virtual check-ins are for existing patients, and they run through our patient portal app, APPatient by Modernizing Medicine. You choose a provider, show us where the concern is with two photos, and answer a few questions about it. A provider reviews your photos and history, then messages you in the app with recommendations or prescriptions as needed.

Three steps get you there. **If you have logged in to the patient portal before, start at step 2.**

A visit fee applies to each virtual check-in.

Step 1

## Activate your patient portal

- Our staff activate your portal for you. You will get an email with your portal username and the practice URL.
- In that email, select Activate Account.
- On the portal website, confirm it is you with your date of birth and last name, then select Verify Information.

Step 2

## Get the APPatient app

APPatient is the patient app from Modernizing Medicine, the company behind our patient portal.

- When the app asks to send you notifications, select Allow. We reply to you in the app, so this is how you hear back.
- Log in with the practice URL coastalfamilyderm.ema.md and the same username and password you use for the patient portal.

Step 3

## Request a virtual check-in

Before you start, check that your medications, pharmacy and allergies are up to date in the app.

- From the home screen, select Request Visit, then Virtual Check-in.
- Choose a provider and the reason for your visit.
- Touch the body map to show where the concern is, then add two photos: one up close and one from farther away.
- Answer the questions about how the concern started and how it has changed. If a question does not apply to you, select Next.
- Confirm whether the medication list we have for you is up to date.
- Review your visit details and photos, select Next, then select Submit.

**What happens next.** Our team reviews your request and, once the visit fee is paid, assigns it to a provider. Your provider reviews your case and messages you in the app with recommendations or prescriptions as needed. Keep notifications on so you see the reply.
